The Rusk County Sheriff's Office said an inmate released to go to a dental appointment on Tuesday morning, April 30, did not show up and is now considered escaped. They are asking for the public's help in locating the man.

David A. Dvorak, 56, of Ladysmith, an inmate with Huber work-release privileges, was released at 7:30 a.m. to go to a dentist in the city of Ladysmith. The dental clinic informed the sheriff's department at 8:30 a.m. that Dvorak did not show up for his appointment.

The Wisconsin Department of Corrections has issued a warrant for Dvorak's arrest and Rusk County will file criminal charges.
